REACH – Women's Throughcare Pathway

Supporting women before and after release

REACH provides continuous, personalised support for women during the final nine months of custody and for up to two years after release.

Delivered across Bandyup, Wandoo and Boronia, the program uses Outcare’s Throughcare model and assertive outreach to address the causes of offending and support lasting connections into their community. Support includes advocacy, practical assistance and connection to housing, employment, family and specialist services.

By ensuring continuity of care across custody and community, REACH helps women build stability, confidence and safer futures.

This pathway supports women to:

  • Transition successfully from prison to community
  • Maintain stable housing and employment
  • Strengthen family and community connections
  • Address the drivers of offending behaviour
  • Reduce the risk of returning to custody
