Jordan* is a 28-year-old Indigenous man whose life has been shaped by trauma, hardship, and time in the justice system. As a child, he faced significant abuse and an undiagnosed intellectual condition, with little support from his family. By his teenage years, Jordan began engaging in petty crimes and spent time in youth detention. At 18, he received a two-year prison sentence for theft. Struggling to protect himself in prison, Jordan became involved in violent incidents, which extended his sentence to a total of ten years.
Three months before his release, Jordan was referred to Outcare. His Case Manager worked closely with him to lay the foundation for a successful transition back into the community. This included emotional support, arranging transitional accommodation, helping him develop a budgeting plan for life after prison, and organising transport from the correctional facility to his new home. During the first few weeks post-release, his Case Manager met with Jordan up to four times a week, providing the consistent guidance and reassurance he needed.
Outcare also supported Jordan with practical steps to rebuild his life. From setting up a new bank account to budgeting for essentials, the program helped him regain independence. Within two weeks of job searching, Jordan secured a retail position, giving him both financial stability and confidence. After four months in Outcare’s transitional accommodation, his Case Manager helped him secure permanent housing through Community Housing, giving Jordan a stable base from which to build his future.
Today, Jordan continues to check in weekly with his Case Manager and is focused on personal growth. He has enrolled in a university course, determined to further his education and create a life beyond the prison system. Through consistent emotional and practical support, Outcare helped Jordan make a fresh start, equipping him with the tools to thrive in the community and pursue his goals with confidence.
