Jacob* is a young Indigenous man whose life has been shaped by multiple challenges from an early age. Living with intellectual disabilities, he began experimenting with substances as a child, and over time became heavily dependent. As a teenager, Jacob and his peers became involved in crime, leading to his conviction and a 12-month prison sentence.
It was during the final three months of his incarceration that Jacob was referred to Outcare’s Community Inclusion program. This program is designed to support people with disability and mental health issues who are at risk in the community, in prison, or following release. Through case workers providing in-home support, accommodation assistance, daily living skills, education, and skill development, the program helps participants build the tools they need to live safer, more independent lives.
At first, Jacob was hesitant to engage. His Case Worker patiently offered guidance, advocacy, and consistent support, helping him gradually open up to the opportunities the program provided. Once he began participating, Jacob made remarkable strides. He was linked with counselling services and support groups that helped him manage his addiction, while also receiving healthcare for substance-related health issues. Daily home visits allowed his Case Worker to address past criminal behaviours and develop strategies for positive decision-making.
Over the following years, Jacob’s dedication, combined with Outcare’s ongoing support, transformed his life. He now lives drug- and crime-free and has recently become a proud father. Jacob has embraced parenting classes, ensuring he can provide a loving and stable environment for his child. Through the Community Inclusion program, Jacob was able to overcome addiction, reduce his risk of reoffending, and access the supports necessary to manage his disability while building a positive future.
Jacob’s story is a powerful example of how personalised support, advocacy, and consistent guidance can turn lives around, creating opportunities for growth, independence, and family connection.
