Finding Hope Through Outcare’s Links Program

At just 16, Hannah* faced challenges that no young person should have to endure. Growing up in a household marked by domestic violence and her parents’ struggles with addiction and mental health issues, stability was a distant concept. Hannah often moved between family members, living with uncertainty and trauma. By the time she was referred to Outcare, she had a history of self-harm, substance dependence, anxiety, and violence. School was no longer part of her life, and she lacked any consistent support from friends or family.

Hannah’s path took a turning point when she was referred to Outcare’s Links program following a drug-related offence. Links is designed to provide voluntary, therapeutic support to children navigating mental health challenges – whether diagnosed or not – who have pleaded guilty to charges in the Children’s Court. Each young person is paired with a Community Support Coordinator, who works alongside a clinical team at the court and coordinates with schools, healthcare providers, and other vital support services.

At first, Hannah was hesitant to engage with the program. But her Community Support Coordinator patiently helped her meet immediate needs – providing food, clothing, transport vouchers, and other forms of emergency relief. Through these acts of support, trust began to grow. Hannah started meeting regularly with her Coordinator, who provided daily guidance, helped her attend medical appointments and treatment, and encouraged healthier routines. Even small steps, like setting up a gym membership, helped Hannah reclaim a sense of control over her wellbeing.

Today, Hannah is on a positive trajectory. She is completing her Year 10 certificate and is on track to graduate from drug court. Perhaps most importantly, she has found motivation, hope, and a renewed sense of possibility for her future. The Links program didn’t just provide support – it provided the structure, trust, and consistent care that allowed Hannah to rebuild her life.

Hannah’s journey is a testament to the power of compassionate, coordinated support. With the right guidance, even young people facing the most challenging circumstances can find a path forward, regain confidence, and embrace the opportunities life has to offer.
